AgeWellAlly
Care doesn't end at discharge. Neither do we.
AgeWellAlly is the patient's personal AI assistant. It is built around their real medical history, and it connects them, their loved ones and their care team as they recover and as they age.
Five things AgeWellAlly does.
Leaving a skilled nursing facility should not mean leaving behind the knowledge and support that helped the patient recover. AgeWellAlly starts with their records and discharge plan and keeps helping after they leave.
A discharge plan that is not forgotten
It turns discharge instructions into steps the patient can follow and check off.
Medicines and appointments
It reminds the patient of the medicines, times and appointments in their care plan.
Help staying out of the hospital
It flags follow-ups and steps the patient has not confirmed.
A health story that keeps growing
The patient can add lab results, notes and, with their permission, recordings of appointments.
Loved ones stay in the loop
With the patient's permission, family and caregivers stay informed, even when they cannot be there.
Our partner Happy People Games runs memory and training games on AgeWellAlly.
It starts in the skilled nursing facility and keeps going.
- 1
At the skilled nursing facility
AgeWellAlly receives the patient's medical history, admission information and discharge plan.
- 2
Going home or to the next setting
Instructions become medicine, appointment and follow-up steps the patient can keep up with.
- 3
The weeks after discharge
Support continues, with attention to missed steps that could lead back to the hospital.
- 4
Every visit after that
The patient can upload records and, with consent, record appointments, so nothing is lost between visits.
As life goes on, the people the patient chooses stay connected, and the health story keeps growing.
Instructions tell patients what to do. AgeWellAlly helps them keep doing it.
Discharge instructions
Today they are handed over once. AgeWellAlly turns them into steps the patient keeps following.
Medical records
Today they record what happened. AgeWellAlly carries that information into every next visit.
Families
Today they struggle to stay informed. AgeWellAlly keeps them connected, with the patient's permission.
The patient decides who is invited.
AgeWellAlly is built around the patient's choices about who sees what.
- The patient decides which family members and caregivers can see their information.
- Appointments are recorded only with consent.
- Families see only what the patient and the care team allow.
- When a medicine or follow-up reminder is not confirmed, AgeWellAlly reminds the patient again and can let the people they have chosen know. It does not replace the care team's own follow-up.
The details
The patient's medical history, admission information, discharge plan, and new records the patient adds.
A personal assistant for the patient, a view on the family's phone and a screen for the care team, in every care setting.
- Who buys it
- Post-acute and senior care providers, and families who keep it at home.
- Works with
- Case Management, Admission Assist
Is AgeWellAlly only for the time after discharge?
No. It starts at the skilled nursing facility and stays with the patient at home, in assisted living, with home health and as they age.
Does it reduce readmissions?
It is designed to help patients avoid preventable readmissions by catching missed follow-ups and steps. We do not yet publish outcome results.
Who can see the patient's information?
Only the people the patient chooses, and only what the patient and the care team allow.
Are appointments recorded?
Only with consent.
What happens after discharge?
The facility sponsors AgeWellAlly during the stay. After discharge, the family can keep it on a monthly plan, so the patient keeps the same assistant and history.
